What to expect on the IMAT Medicine

Overall difficulty

Challenging — requires deep knowledge of cell biology, molecular genetics, organic reactions, stoichiometry, and quantitative reasoning.

Question style

Multiple-choice (5 options, 1 correct). Questions test conceptual mastery, chemical equation balancing, genetics probabilities, and logical deductions.

Time per question

1.67 minutes per question (100 min total for 60 questions). Pace yourself carefully through Biology and Chemistry.

Exam-day tips

1The IMAT penalizes wrong answers by −0.40 points (and awards +1.5 for correct answers). If you cannot eliminate at least 2 wrong options, leave the question blank!
2Biology accounts for 23 out of 60 questions (~38% of your total score) — master genetics and cell transport first.
3Aim for 50+ total points out of 90 to secure non-EU or EU medicine seats at top universities (Milan, Bologna, Sapienza, Pavia).
